GuardianRx Patient Care System
Guardian provides the most comprehensive Windows patient charting system with multiple
categories of data entry including Clinical, Documentation, and Monitoring. Two complete
CarePlan templates for Asthma and Diabetes provide a consistent means to screen and enroll
patients in patient-specific care plans.

CarePlans
Two pre-defined CarePlans are available to enroll patients into a custom-tailored
Asthma or Diabetes CarePlan. Responses to screening questions help determine the exact
issues, goals, and activities that will be used to create the plan. The National Institute
of Health and the American Diabetes Association guidelines provide the guidelines for the
Asthma and Diabetes CarePlans, respectively.

Monitoring
The Monitoring facility offers a comprehensive means of tracking vital signs, labs, and
indicators that are specific to a patient’s health concerns. Over 65 monitoring items may
be selected for each patient and tracked indefinitely. Categories of monitoring items include
Vital Signs (with a complete pain assessment), Chemistry, Hematology, Coagulation, Microbiology,
and Indicators and Measures. Accumulated data can be plotted on bar charts and line graphs and
then printed/faxed to patients and/or physicians.

Immunizations
Many pharmacists are making positive contribution to their community by offering a vaccination
service. Whether you are administering the vaccines or contract with a health agency to come
into your store and administer them, Guardian has a convenient immunization record for you to
keep track of those patient’s you have served.

Documentation
Guardian offers an extensive documentation facility that is used to record patient visits and
events. Documentation data is entered using simple point-and-click actions while an automatic
text generator does all the work. Customized SOAP notes can be entered and printed/faxed to
patients, physicians, and insurers. Fees generated from your services can be generated onto a
basic invoice or to a CMS 1500 form. CMS 1500 Forms As patient visits and encounters are
documented, Guardian is ready to print the CMS (HCFA) 1500 form as a means of requesting payment
for your patient care services. Billing output is printed directly to the form and the data can
be modified/edited—on screen—right up to the point of printing.

Clinical Simulation
GuardianRx’s Clinical Simulation facility provides a forum to answer "what if" questions. To
this extent, patient profiles may be analyzed and screened using this facility. Clinical
Simulation uses two types of screening: 'Prospective', which compares any new items entered on
the profile against the other items, and 'Retrospective', which compares all items on the
profile, including the new items.

Brown Bag
The brown bag module employs the power of the DUR database to analyze patient profiles that do
not exist in the GuardianRx database. Brown Bag provides for inputting additional patient meds
from other sources, therefore making possible a complete DUR analysis of the patient’s full
array of medications. This can be especially useful for patients that you may encounter offsite,
for instance, at health fairs or assisted living settings or through “brown bag” sessions you
may offer at your store.
